Home alone permission
Parents and carers of children in Year 6 and 5 (summer term only) may request permission from the headteacher for their child to travel independently to and from school. Parents must wait for a response from the school to confirm permission has been granted.
Out and about permission
From time to time West Grove staff may wish to take children out of school at short notice. This will be to explore the local area during the school day. Please complete this form to give your permission.
Medication administration
The school will not be able to give your child medicine unless this form is completed. Please note we can only accept medication prescribed by a doctor.
Collection arrangements
Please use this form to advise us of named adults who can collect your child. Once you have completed this form our records will be updated.
Photograph / video permission
West Grove likes to celebrate your child’s effort and achievements, as a result images may be used on the school website, social media or to share school events with parents such as assemblies and performances. If you would prefer us not to include your child please complete the opt-out form below.
